How To Change Tax Withholding, Personal Info & Where Your Check Is Sent

(This information applies to academic and staff employees who are not students.)

Changing your withholding

To change your withholding, you must complete a new W-4 form (PDF). See General instructions for W-4(PDF) ; specific Federal and State instructions may be obtained from the Central Payroll Office (459-2488) or from your Payroll/Personnel Assistant. When complete, the form may be returned in person or via campus mail to the Physical and Biological Sciences Business Office or to the Central Payroll Office at Accounting, University Business Park.

Alternatively, on-line changes to your tax withholding may be made at the UC For Yourself web site. You will need your individual login and PIN. On-line changes take 24 hrs. to become effective.

Changing your personal information
(name, address, phone, etc.)

To change any of your personal information, complete and sign a Personal Data Change form (PDF or Word 98 ) and return it to the Physical and Biological Sciences Business Office, or you may send e-mail to your Personnel/Payroll Representative.

Changing where your check is sent

You may send e-mail to your Personnel/Payroll Representative in the Physical and Biological Sciences Business Office or download and complete the Check Disposition Form (PDF or Word 98), then sign and return it to the Physical and Biological Sciences Business Office.
