Performance Evaluation Form

In accordance with Personnel Policies for Staff Members (PPSM) and collective bargaining contracts, supervisors are responsible for completing at least one performance evaluation during an employee's probationary period and, thereafter, an annual performance evaluation for any career staff employees under their supervision. Employees in limited (formerly called casual) appointments may be evaluated at any time the supervisor believes a written evaluation would be beneficial.

Performance evaluation is intended as a means of measuring and enhancing individual performance, fostering professional development and career growth, and providing documentation for merit increases and other personnel actions. Annual written performance evaluations give supervisors and employees an opportunity to discuss: job responsibilities, performance over a specific period of time, any problems which may have arisen, and future objectives for development. Evaluations should be sufficiently specific to inform and guide the employee in the performance of her/his duties. However, written performance evaluations are not intended to replace the continuing relationship between supervisor and employee. Supervisors are expected to meet with employees throughout the review period to provide ongoing feedback on performance.

The Division has developed performance management guidelines Rev 6/03 (PDF) to assist supervisors with preparing thorough and meaningful evaluations and conducting performance evaluation meetings with employees. Supervisors are strongly encouraged to familiarize themselves with these guidelines.

The performance evaluation form may be completed in a variety of ways (see options). The form is meant to provide an outline to evaluate performance on established goals/objectives and ongoing responsibilities relative to general competencies.

The call for annual performance evaluations is sent to supervisors in June of each year. Evaluations are due back to the Staff Human Resources by the end of August.
