Administration > Business Office > Personnel/Payroll > Staff Employees Performance Evaluation Form In accordance with Personnel Policies for Staff Members (PPSM) and collective bargaining contracts, supervisors are responsible for completing at least one performance evaluation during an employee's probationary period and, thereafter, an annual performance evaluation for any career staff employees under their supervision. Employees in limited (formerly called casual) appointments may be evaluated at any time the supervisor believes a written evaluation would be beneficial. The Division has developed performance management guidelines Rev 6/03 (PDF) to assist supervisors with preparing thorough and meaningful evaluations and conducting performance evaluation meetings with employees. Supervisors are strongly encouraged to familiarize themselves with these guidelines. Also available are some tips on how to download and enter data into the evaluation form which supervisors may find helpful.
