  1. Invitation to orientation session & campus map
  2. Physician Designation form
  3. Form to be completed for campus phone directory listing
  4. New Employee General Information List (tells employee what information they can get from their supervisor, Staff HR, or Labor Relations
  5. List of holidays/working hours in the month
  6. "If you have a disability" flyer
  7. TAPS (parking) application
  8. UCSC Collective Bargaining Units (representative's name, phone number and the "fair share" monthly payroll deduction information)
  9. Benefits packet, if applicable
  10. Plan Coverage for Safe Harbor, for non-career employees ONLY
  11. University policies on substance abuse and dependency, for a smoke-free environment, and for Sex Offense/Harassment
  12. UCSC Ombudsman, Equal Employment Opportunity/Affirmative Action, Environmental Health and Safety (EH&S), and Illness and Injury Prevention Program (IIPP) brochures
  13. Crime Awareness and Campus Security Report

BY PHYSICAL AND BIOLOGICAL SCIENCES' PERSONNEL/PAYROLL STAFF TO ALL EMPLOYEES

  3. Paycheck release date, including when to expect the first check

  7. Background Investigation forms (if applicable)
  8. Time and attendance information:
    • time record information on how to complete, when to return and who is to sign time record*
    • for employees working a nonstandard work week, information for completing time record during months with holiday
    • overtime procedures* including Notice Regarding Overtime Compensation (for clerical employees)
    • consequences of working less than 50% time during probationary period*
    • waiting periods for taking vacation, if appropriate*
    • December closure information* (if known)
