Equipment Disposal Procedure (Lab refrigerators/freezers/other equipment) 
REQUESTER/USER RESPONSIBILITIES
The user is required to notify PBSci Facilities of ALL disposal transactions and clear the equipment with EH&S prior to transferring to Surplus or having it removed for disposal. The user is also responsible for filing a Disposal Request (DR) and a EIMR if necessary, and returning them to PB Sci Facilities.
1.0 Sale/Surplus Option
The User has the option of selling the equipment instead of disposing of it. Note that no refrigerator, freezer, microwave or other potential consumer items used in a laboratory may be resold to the general public. The UCSC Surplus Operations is responsible for all aspects of sale, including advertising, shipping, and storage of equipment during sale. 1.1 User Responsibilities
A. Completion of DR and/or EIMR forms
A DR (Disposal Request) must be completed for all equipment disposals, transfers or surplus sales.
Completed and signed DR forms must be turned in to the PBSci Facilities office (214 PSB)for processing prior to the equipment being disposed of.
Keep a copy for yourself.
An EIMR (Equipment Inventory Modification Request) must be completed for inventorial items (these items will have a an affixed UCSC property tag). The EIMR form is used to report equipment "scrapped" or "transferred" which is valued over $5000 (original purchase price). Items under $5000 do not require paperwork for disposal, unless they have a UCSC property tag. Completed and signed EIMR forms msut be be turned in to the PBSci Facilities office(214 PSB) for processing. Keep a copy for yourself.

C. Removal of contents/Cleaning Guidelines
Remove any and all contents from refrigerator/freezer.
(If used for radioactive storage, please contact EH&S. (John Pasinosky, johnppas@ucsc.edu, 9-3911, and complete EH&S radioactive survey).
Clean and wipe down all interior/exterior surfaces in compliance with EH&S standards.

Remove ALL existing labels (warning, caution, chemical, radiation, hazard, etc.). Label removal should occur only after EH&S has cleared equipment and should include UCSC Property/Inventory Sticker.
1.2 PBSci Facilities Responsibilities
PBSci Facilities will initiate contact with EH&S for clearance of equipment. EH&S will e-mail clearance for equipment to PBSci Work Order Desk (pbsciwodesk@ucsc.edu).
A recharge order will be initiated to Central Receiving for removal of equipment.
EH&S clearance will be attached to recharge form.
PBSci Facilities will post notice that equipment has been cleaned,
Once EH&S has provided clearance, and all stickers have been removed, PBSci Facilities will provide a recharge number for Central Receiving.
PBSci Facilities will pay disposal costs.
